WHAT IS CASE MANAGEMENT?
Case management refers to a comprehensive approach to planning, coordinating and managing the care and support required by people dealing with complex health conditions or unique care needs. Case management focuses on improving the person’s overall well-being and achieving the best possible health outcomes whilst seeking to optimise the budget available.

WHAT ARE CASE MANAGERS?
Case Managers are appointed to help coordinate and oversee a person’s care package. These skilled professionals specialise in managing, coordinating and overseeing the care and support of people with complex medical conditions or care requirements. They act as advocates for their clients, working to ensure that all aspects of their physical, emotional, and social well-being are considered and addressed.
Case Managers collaborate with various care providers, multidisciplinary teams, caregivers, and support services to create comprehensive and cost-effective care packages that meet their clients’ needs. Their role involves careful planning, ongoing communication, and continuous evaluation to adapt the care plan as a client’s circumstances evolve. Clients are usually referred to Case Managers through their healthcare providers or social workers.

WHAT DOES OUR NURSE-LED CARE SERVICE INCLUDE?
Our service is nurse-led and carers are trained in the clinical skills required to provide care and support for complex, terminal and other medical conditions. They are supported by an experienced, dedicated registered nurse who undertakes regular competency assessments to ensure high standards of care delivery and who provides clinical monitoring, supervision and support.
With this leadership and oversight carers can perform many delegated healthcare tasks and clinical interventions at home, negating reliance on community district nursing teams, including:
Respiratory and airway support
- Oxygen delivery and suctioning
- Cough assist working in partnership with physiotherapists
- Tracheostomy management
- Non-invasive Positive Pressure Ventilation (NIPPV) via masks – CPAP/BiPAP
- Invasive ventilation via machines – Nippy
Feeding and supplementary support
- NGT/PEG/gastrostomy management
- Continuous/intermittent pump feeding
Elimination and toileting
- Catheter care
- Stoma care
Mobility
- Skin integrity and pressure area care
- Assistance with mobility aides to encourage safe moving and handling
HOW WE PROVIDE COMPLEX CARE AT HOME
Our trusted complex care service is flexible and can be provided in the following ways:
- Hourly care or visiting care is flexible and can be provided for as many hours as required throughout the day or week to ensure our clients are never without the support they need.
- Respite care for those needing a short-term arrangement after hospital discharge to rehabilitate or when a family carer needs a break from their caring role.
- 24 hour care for people with substantial care needs who need round-the-clock support to live well at home.

We know through our existing partnerships with Case Managers this clinical excellence provides reassurance, drives efficiencies in ways of working and delivers best-practice clinical care to those we care for. Our nursing teams are experienced in providing high levels of accurate reporting, saving you valuable resources and ensuring continuity of care.
We also work collaboratively with other healthcare professionals, multidisciplinary teams, therapy providers and community support services involved in our client’s care to ensure an integrated and holistic approach to the nursing care we provide. The level of care provided is planned in partnership with all health professionals involved in a client’s care, as we know through our experience this ensures goals are reached and outcomes are improved.
HIGHLY TRAINED AND EXPERIENCED CARERS
Our dedicated carers are experienced and expertly trained to provide the specific clinical care and support required by people with complex care needs.
All carers providing complex care packages have at least six months of care or health sector experience. We adopt a bespoke and ongoing approach to training that ensures our carers have the knowledge, skills and aptitude to deliver high-quality complex care that meets the very specific needs of our clients. Through our unrivalled supervision and clinical oversight, carers can adapt their approach and evolve the care they provide to meet changing needs.
Our competent carers can deliver clinical interventions that negate the need to rely on community district nursing teams. This allows them to work efficiently and to achieve more in less time, providing funding savings back to our commissioning partners.
